Registration

You may register for your test, procedure, or admission on the day you arrive by going to the Registration Desk near the Emergency Room entrance.

Important Documents to Bring

Please bring the following items with you when registering:

Insurance Cards
Picture identification
Social Security number
Order from a Physician for the test or procedure you will be having
A form of payment
