Schedule Your Wellness Visit at Physicians’ Health Group at No Cost to You!
What to Expect:
During your wellness visit, we will identify any health risks you may have and discuss Medicare-covered services that allow your healthcare team to monitor your health conditions closely to lower your risk of illness or injury. Our goal is to help you reach your goals of getting and staying healthy.
Your visit will include:
- Routine measurements (like height, weight, and blood pressure).
- A review of your medical and family history.
- A review of your current prescriptions.
- Personalized health advice.
- Advance care planning.
Eligibility:
After having Medicare Plan B for 12 months or longer, you are eligible for yearly wellness visits at no cost to you.
If the provider decides to conduct additional testing, you may have to pay co-insurance, and the Part B deductible may apply. Should Medicare not cover additional tests and/or services, you may have to pay the full amount.
